Participant Waiver

Anyone attending any PTA Sponsored event must have a participant waiver on file. If you have not filled out this form yet, please click on the form to the right to submit electronically.

Each member of your family must be listed on the form, and a new form must be filled out for each school year.

A paper copy was sent home on the first day of school as well.

Parent's Approval, Student, Family, and Participant WaiverType the name of all family members who may participate in any PTA sponsored events for the 2024-2025 school year (including student, siblings and parents).

Teacher Appreciation Week

Otherwise, you can make a monetary donation that will go towards the food we will provide each day.

To make a monetary donation, please click here: Susan B. Anthony Elementary PTA to donate online, or if you prefer sending cash, you can do so in an envelope marked "PTA" and delivery to your child's teacher or to the front office by Friday, May 2nd.

**All physical donations must be dropped off prior to 10:00 on the morning of your donation day**

Monday - TK/Kinder Parents host Taco Bar.

Tuesday - 1st/2nd Grade Parents host Pasta Bar.

Wednesday - 3rd/4th Grade Parents host Sandwich Bar.

Thursday - 5th/6th Grade Parents host Pizza/Salad.

Book Fair

Thank you to all that purchased books at our book fair! We had a great turnout to our Read Across America event on Wednesday night, and a very successful book fair! (Sorry students that we ran out of erasers...). Proceeds from the book fair will go to our teachers to purchase supplies and other needs for their classrooms!

Fall Festival

Fall Festival is always a big hit! Thank you for everyone who joined us this year for trunk or treating, bounce houses, gaming trucks, food, and fun!!! Thank you to all who volunteered at the festival and who hosted a trunk!

Winter Festival

Thank you so much to all of our volunteers, vendors, and the Corona Fire Department! There was such a big turnout to this event. We hope everyone had fun playing carnival games, shopping for holiday gifts, and playing in the snow!

Thank you to all who donated toys to the Spark of Love toy drive!

Skate Night

Skate Night was a blast! Thanks to all who joined us! Everyone had so much fun! Thank you to The Rinks - Corona Inline for lending us your facility!

Magic Show

Thank you to all who came out for our first Magic Show! It was such a fun and entertaining evening full of magic!!! Thanks to Anthony the Magic and his lovely assistant! They were great!

Book Fair Family Night

Thank you to all who joined us for our book fair family night, and to those who purchased a book at our book fair!! All proceeds will go back to the classrooms for supplies and activities!
